Eligibility and Selection Procedure for MHA in Sanjay Gandhi Postgraduate Institute of Medical Sciences (A) Qualification: MBBS/ BDS from an institute/ college recognized by MCI/ DCI, Govt of India. (B) Age: The upper age limit for admission is 35 years as on 31st July 2021. Upper age limit is relaxable as per Govt of UP rules in force at the time of admission. (C) Fee: The applicants are required to enclose a non- refundable bank draft of any nationalized bank for Rs.1000/- in favour of "Director, SGPGIMS (Academic A/c), Lucknow", payable at Lucknow. (D) Mode of Selection: Entrance Examination: MCQ based theory paper in English medium only. |

- Printed application form in 3 pages (2-page form + 1 page of identification-cum-attendance card) with your signature at two places (page 2 of the form and on the identification-cum-attendance card, page 3), and paste self-certified, recent, passport-size, coloured photographs at appropriate places.
- A self-certified copy of matriculation/age-proof certificate.
- Proof of MD /MS / DNB course recognized by Medical Council of India / State Medical Council and a self-certified MCI recognized MD/ MS/ DNB degree / passing certificate. Applicants who are likely to complete the 3-year requisite tenure and degree by current year (from an MCI recognized program only) will be provisionally permitted subject to production of a proof of such completion from competent authority. MD / MS / DNB teaching program (i.e. Department or Institution) which is an essential qualification must be recognized by the MCI as on date.

The filled-in Application form with requisite fee is to reach, The Executive Registrar, SGPGIMS, Raebareli Road, Lucknow - 226014 along with all relevant documents in support of age, qualification, proof of recognition of professional degree from the relevant professional council, experience, reservation, etc and duly signed upon by the candidate before the last date that is 10th August 2021. Envelope should be superscribed with the Advertisement Number and "Application for Entrance Exam to MHA Program for 2021".
