Application Procedure for MSc in Tata Memorial Centre Applicants can obtain application online and free of cost. Forms can be downloaded from the website. Completed and signed application form with attached documents and demand draft payable at Mumbai for Rs. 2000/- (Rupees Two thousand only) drawn in favour of “Tata Memorial Centre, Parel, Mumbai”. should be sent to the Office of the Director, Academics, Homi Bhabha, Tata Memorial Hospital, Parel, Mumbai by registered / speed post only. Instructions to fill up the form: Candidates are advised to save the completed application and print the document on one side of the paper only. Online submission ALONE will NOT be considered.· The completed Hardcopy of application should be signed and sent by registered post / speed post to the Office of the Director – Academics,TMC, 13th Flr, Homi Bhabha Block Building,Tata Memorial Centre,Dr. Ernest Borges Marg, Parel,Mumbai – 400 012Copies of all relevant certificates and documents should be attached.· The envelope containing the application should be marked “APPLICATION FOR M.SC.
