Filled application forms shall be submitted in person to the

Andhra Pradesh on or before due date by post.
Applications unaccompanied by required certificates or applications with incomplete entries and ineligible applications shall stand rejected automatically. Please do not leave any column blank. Where information is NIL write NO / NIL.
Applications of the candidates, who furnish incorrect information, enclose false / incorrect certificate or approach through agents shall stand rejected automatically.
Application shall be filled in English by the candidate in his / her own handwriting.
The cover must be super scribed “For Admission into I year MDS course – 2017 - 2018”
Only for candidates who have downloaded the application form from the website must attach an Demand Draft Amount Rs.3,000 for along with this application. Please provide the details here

- Certified copy of the SSC or any equivalent examination showing date of birth and other particulars of the candidate
- Copy of Intermediate Marks Memo
- Permanent or Provisional Degree certificate.
- BDS Registration from state dental council.
- Migration certificate.( If UG is not from DRNTRUHS).
- DR NTRUHS MDS ENTRANCE TEST Hall Ticket and Rank Card for ascertaining merit
- GAP Certificate After UG (If any)
- Community, Nativity Certificate
- Transfer Certificate of BDS
- Photographs with Name & D.O.B.
- Bank Guarantees as per the Category of Admission
